Carnosine supplementation improves glucose control in adults with pre-diabetes and type 2 diabetes: A randomised controlled trial

Hariharan R, Cameron J, Menon K, et al.. Nutrition, Metabolism and Cardiovascular Diseases, 2024; 34(2):485-496.

Randomised controlled trialHumans

Design
Randomised controlled trial
Subjects
43 adults with prediabetes or type 2 diabetes (30% female)
Dose used in the study
2 g carnosine per day
Duration
14 weeks
What was measured
Primary outcome: glucose metabolism on a 2-hour 75 g oral glucose tolerance test. Secondary: body composition (DEXA), calf muscle density (pQCT), anthropometry.

What the authors reported

Carnosine lowered blood glucose at 90 minutes (-1.31 mmol/L, p = 0.02) and 120 minutes (-1.60 mmol/L, p = 0.02) and reduced total glucose area under the curve (-3.30 mmol/L, p = 0.04) compared with placebo. Insulin levels did not change. No secondary outcome changed. The authors said the result supports larger trials.

Limits of this study

Small trial (43 participants). Fourteen weeks. Insulin unchanged, so the mechanism is unclear; the authors suggest changes in hepatic glucose output but did not test it. The carnosine was supplied by Flamma Group, which the authors say had no input into design, conduct, analysis or reporting. Full text not read.
